This form is to be filled out within 30 days of home purchase.
In order to transfer warranty to 2nd property owner, this form must be completed and a money order or certified check must be mailed and made payable to: Polaris Windows & Doors for $100 to cover warranting transfer handling costs.

Your order number can be found on the interior of your window frame on a warranty sticker. All windows in a single order will have the same number; you only need to locate one sticker to get this information.
